HSTA Waiver Information

HSTA Waiver Maintenance Form - This form is required to be completed by EVERY HSTA graduate by March 1st each year AND when Transferring between colleges. It is used by HSTA to notify your college of your need to accept or decline the HSTA Undergraduate, Graduate, or Health Professions Fee Waivers.
