Registration
No later than Friday, September 24, 2004, at the HSTA Fall Workshop. Mail or hand to J Bardwell, the completed registration form with your enclosed fee (see below) to the WVU Extended Learning Office at:

Obtaining and Completing the Application/Registration Form : The

. If you have trouble obtaining the form or completing
it, email the instructor (
jbardwell@hsc.wvu.edu ) stating your problem
and she can fax to you the registration form or attempt to trouble
shoot your questions as needed.
Make sure to complete BOTH sides of the form. Side two, section II is where you will enter the following information to register for THIS course (CI 694K):
- Course CRN#: 86318
- Subject: C&I
- Course # : 694E
- Section #: 6AA
- VR CR Hrs: 3
- Grd Opt: 2
Attach your check or money order payable to WVU in the space shown at the bottom of page two OR provide your credit card information if paying by credit card. The total cost is $99 (this is for fees only, tuition is waived) unless you are not currently at WVU graduate student (see immediately below).
If you have not been accepted as a WVU graduate student, you may take this graduate course only with the permission of the Instructor and this permission can only be granted once in your career, i.e., you can take only one course for graduate credit at WVU as a non-WVU student. You also must pay an additional $10 "Professional Development" fee (therefore your total fee is $109 as opposed to $99). Email jbardwell@hsc.wvu.edu with your request before registering. If you plan to take additional graduate credit courses at WVU, you should also apply soon to WVU for graduate admission.
